The 3rd Hokage has been a Hokage for quite some time. He can often be seen smoking.
A hokage is a very honorable title given to the leader of a village. Sandaime 3rd Hokage is that leader. He rules of the Hidden Leaf Village, Konoha.
The chinese symbol on his hat means "Fire." This is because that the Hidden Leaf Village is situated in the Fire Country. There are also many other kages. For example, there's the kazekage. The kazekage rules of the Hidden Sand. The Hidden Sand is located in the Wind Country. "Kaze" means wind. The kazekage has a wind symbol on his hat.
